There is also a new app that is available for Apple and Android smartphones as well as tablets. This app enables you to take photos of damage to your vehicle (low-impact damages only: bumper dents, scratches etc. The vehicle must still be drivable.) and upload them to get your estimate within eight business hours.

Also, an employer must file the eFROI (electronic First Report of Injury) using the 18/10 rule. The eFROI must be filed within 18 days after the injury/illness event or within 10 days of the employer’s knowledge of a work-related injury/illness, provided the injury has caused or will cause the injured employee’s loss of time from regular duties of one day beyond the workday or shift during which the accident occurred, or has required or will require medical treatment beyond ordinary first aid or more than two treatments by a person rendering first aid. Note filing an FROI-00 will satisfy the employer’s filing requirement. Filing a paper C2F will not satisfy the WCB filing requirement. NYSIF policyholders should use eFROI to file the FROI-00.

Safeco allows online claim filing, and mobile app filing, for Auto, Home, Condo and Rental policies.

ShelterPoint does not allow claims to be submitted online or by phone at this time. Click here to download the claim report form: Search under New York State, NY DBL/PFL, Claim Forms and download the DB-450 for a disability claim, and the IH-007 if the employee is in the hospital. For Paid Family Leave claims, utilize the PFL form applicable to the type of leave being requested.
You can fax claim information to 516-504-6414. Or mail to the address on the form. Be sure to reference either your policy or claim number.
If you are self-employed and making a claim for yourself, note that your accountant should sign in the employer section to verify your income and you should attach your latest Tax form 1040 Schedule C, without that, they may request the whole tax return as proof of your lost income. PFL will need to be pre-approved.
